Project Summary (2-3 pages)
1. Project Title

3. Project Implementation Location(s): Please specify the exact location(s), including the Name of the Local Government Area (LGA), Ward, and Community/Location.
4. Statement of the Problem to be Addressed and Proposed Project Duration
5. Project Goal and Specific Objectives, including Clearly Defined Targets
6. Detailed Description of All Planned Project Activities
7. Expected Outcomes of the Project, with Explanations
8. Identification of Key Stakeholders from Government and the Beneficiary Community, including a Description of Their Roles and Involvement
9. Sustainability Plan: Explanation of How the Project Will Be Maintained After the Grant Period Concludes
Financial Summary (1-2 pages)
1. Provide a detailed and realistic cost estimate for all proposed project activities.
2. Include a budget provision to cover our monitoring visits
3. Present the total project budget, clearly indicating the amount being requested.
4. Disclose any additional sources of funding (if applicable) and specify the expected timeline for securing such funds.
5. State the organisation’s total annual budget for the previous two (2) financial years.
Project Management & Risk Mitigation(1 page)
1. Identify all anticipated project risks and provide a detailed explanation of the proposed mitigation strategies.
2. Describe the monitoring and evaluation framework, including the roles of the organisation and relevant stakeholders in overseeing project implementation.
3. Provide any additional information considered relevant to the effective management and successful delivery of the project.
